Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. You will assist with daily living activities, mobility, meals, and medication management within ISP/IPP plans, under supervision.

The role requires strong documentation, teamwork, and a commitment to dignity, independence, and inclusion in community settings. Field-based with flexible hours and travel as needed.

Qualifications

• English proficiency required (spoken and written).
• Preferred: bilingual abilities.
• High school diploma or equivalent.

Responsibilities

• Provide hands-on, person-centered support in home and community settings.
• Assist with daily living activities, meal preparation, mobility, and medication as authorized.
• Implement and monitor ISP/IPP and behavior support plans under supervision.
• Maintain accurate documentation and timely service notes, observations, and shift verification.
• Support independence, safety, and community participation for individuals served.
• Attend trainings and team meetings; adapt to staffing needs and regulatory standards.
