Job summary

• Assist with daily living activities including personal care, meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies under guidance of the interdisciplinary team.
• Monitor health, safety, and emotional well-being; promote independence and community participation.
• Maintain structured routines; support transportation to appointments and community activities.
• Participate in trainings and team meetings; document accurately and timely.
