Job summary

OneWell Health Care is seeking a dedicated Direct Support Professional (DSP) / Direct Support Worker to provide hands-on, person-centered care to individuals with IDD in home and community settings in Pennsylvania. Flexible schedule and field-based work support independence and participation.

You will assist with daily living, meal prep, mobility, medication reminders, and documentation while following ISP plans and HIPAA guidelines.

Qualifications

• HS diploma or equivalent required.
• Experience in caregiving or home health is preferred.
• English proficiency required, bilingual a plus depending on needs.

Responsibilities

• Assist with personal care and daily living activities.
• Support meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well-being.
• Maintain structured routines and documentation; participate in trainings and team meetings.
• Provide transportation and community participation support as needed.
