Job summary

OneWell Health Care seeks a Direct Support Professional (DSP) to provide person-centered care for individuals with IDD in home and community settings. You will support daily living activities, health monitoring, and community participation under ISP/IPP guidance.

Role emphasizes hands-on care, documentation accuracy, and participation in trainings, with flexible scheduling and field-based responsibilities in Pennsylvania.

Qualifications

• High school diploma or equivalent.
• Preferred: caregiving/home health/direct support experience with IDD.
• Experience in community-based or residential support settings.
• English proficiency; bilingual skills a plus depending on program needs.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with personal care, daily living activities, meals, mobility and medications as authorized.
• Implement behavior support strategies and monitor health, safety, and well-being.
• Maintain documentation, participate in trainings and team meetings.
