Job summary

OneWell Health Care is hiring a Direct Support Professional (DSP) to support individuals with IDD in home and community settings across Pennsylvania. The role emphasizes person-centered care, daily living assistance, and health monitoring.

You will implement ISP/IPP plans, assist with transportation and daily routines, and participate in trainings while maintaining accurate documentation and HIPAA compliance. A flexible schedule and field-based assignments are expected.

Qualifications

• High school diploma or equivalent.
• Experience in caregiving or home health preferred.
• Ability to follow care plans and documentation guidelines.

Responsibilities

• Provide hands-on, person-centered support in home and community settings.
• Assist with daily living activities, meal preparation, mobility, and medication support as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well-being.
• Maintain documentation, participate in trainings, and support scheduling with flexibility.
