This field-based role requires reliable transportation, English proficiency, and ability to work flexible hours, including evenings and weekends.

Qualifications

• High school diploma or equivalent.
• Prefer experience in caregiving or direct support roles.
• Experience in community-based or residential support settings.

Responsibilities

• Provide hands-on, person-centered support per care plans.
• Assist with daily living activities, meals, mobility, and medications as authorized.
• Support transportation to appointments and community outings.
• Maintain accurate and timely documentation and comply with care plans and regulatory standards.
• Participate in trainings and team meetings.
