Job summary

OneWell Health Care in Pennsylvania is seeking a compassionate Caregiver / Direct Support Professional (DSP) to support individuals with IDD in home and community settings. The DSP will assist with daily living activities, transportation, meal preparation, and health monitoring, following individualized service plans while documenting care and outcomes.

The role is field-based with flexible scheduling, requiring reliability, strong communication, and adherence to HIPAA and regulatory standards.

Qualifications

• High school diploma or equivalent.
• Experience in caregiving, home health, or direct support preferred.
• Ability to use electronic documentation and EVV systems.
• English proficiency required; bilingual is a plus.

Responsibilities

• Assist with personal care and daily living activities.
• Support meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well-being.
• Maintain accurate documentation and ensure compliance with care plans and regulatory standards.
• Participate in trainings and team meetings to maintain quality of care.
