Job summary

OneWell Health Care seeks a dedicated Caregiver / Direct Support Professional (DSP) to provide hands-on, person-centered support for individuals with IDD in home and community settings. The role emphasizes daily living assistance, health monitoring, and community inclusion, guided by ISP/IPP plans and a multidisciplinary team.

The DSP will deliver respectful, high-quality care, promote independence, and maintain accurate, timely documentation in a field-based environment with flexible scheduling.

Qualifications

• High school diploma or equivalent.
• Prior caregiving experience preferred.
• Experience in community-based or residential support settings is a plus.
• Ability to use electronic documentation and EVV systems.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety and emotional well-being.
• Maintain structured routines, support employment or vocational activities, and document accurately.
