Job summary

OneWell Health Care seeks a dedicated Direct Support Professional to provide person-centered care to individuals with IDD in home and community settings in Pennsylvania. The DSP delivers daily living support, skill-building, health and safety monitoring, and promotes independence and community participation.

Role requires adherence to care plans, HIPAA, and regulatory standards while documenting activities and coordinating with the interdisciplinary team.

Qualifications

• High school diploma or equivalent required.
• Experience in caregiving, home health, or direct support preferred.
• English proficiency required; bilingual abilities a plus depending on program.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home/community settings.
• Assist with daily living activities, meal prep, mobility, and medication as authorized.
• Maintain documentation, ISP plans, and adherence to regulatory standards.
• Support transportation and community participation; participate in trainings and team meetings.
