Job summary

OneWell Health Care in Pennsylvania seeks a dedicated Caregiver / Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. This field-based role emphasizes daily living assistance, health monitoring, and community inclusion while promoting independence and wellbeing.

You will implement ISP/IPP guidance, document care activities, and collaborate with an interdisciplinary team.

Qualifications

• HS diploma or equivalent required.
• Experience with IDD preferred.
• Ability to follow care plans and documentation guidelines.

Responsibilities

• Assist with daily living activities and personal care.
• Support meal prep, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health and safety.
• Maintain structured routines and support employment or vocational activities.
• Document care activities accurately and timely, and participate in trainings.
