Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide hands-on, person-centered care for individuals with IDD in home and community settings. You will support daily living activities, health monitoring, and community participation under supervision.

The role emphasizes independence, dignity, and compassion, with flexible scheduling and a collaborative team environment in a field-based setting.

Qualifications

• High school diploma or equivalent.
• Experience in caregiving, home health, or direct support for individuals with IDD preferred.
• Ability to follow written care plans and documentation guidelines.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well-being.
• Maintain accurate and timely documentation and participate in trainings and team meetings.
• Support community inclusion and transportation to appointments and activities.
