Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. The DSP will assist with daily living, health monitoring, and community participation, under supervision, in a field-based role.

The position emphasizes individualized service plans, confidentiality, and compliance with HIPAA. A high school diploma is required, with preferred experience in caregiving and strong documentation

Qualifications

• High school diploma or equivalent.
• Experience in caregiving, home health, or direct support preferred.
• Ability to follow written care plans and documentation guidelines.

Responsibilities

• Assist with daily living activities and personal care.
• Support meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and well-being.
• Assist with transportation to appointments or community outings and documentation.
• Participate in trainings and team meetings; maintain accurate records.
