Job summary

OneWell Health Care is seeking a dedicated Direct Support Professional (DSP) to assist individuals with IDD in Erie, PA. The role focuses on hands-on, person-centered support, daily living activities, health monitoring, and community inclusion in home and community settings.

Responsibilities include implementing ISP/IPP guidance, maintaining documentation, and supporting transportation and community participation with HIPAA-compliant practices.

Qualifications

• English proficiency (spoken and written) required.
• High school diploma or equivalent.
• Flexible schedule and willingness to work field-based care.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meal prep, mobility, and medication as authorized.
• Implement behavior support strategies and monitor health, safety, and well-being.
• Maintain structured routines, document care activities accurately and timely.
• Facilitate transportation to appointments and community activities.
