Job summary

Fresenius Medical Care Des Moines is seeking an entry-level Home Therapies RN CAP 1 to join the clinical advancement program. You will assess, plan, intervene and evaluate care for an assigned group of patients across home dialysis training and treatment, coordinating with the physician and health care team.

As part of the ESKD care team, you’ll participate in teaching, leadership, and quality improvement activities, with on-call coverage and travel to training sites as needed.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• CPR BLS Certification.
• Must meet the practice requirements in the state in which he or she is employed.

Responsibilities

• Provide and coordinate clinically competent care for assigned patients.
• Assess and monitor patient response to home dialysis training and treatment.
• Educate patients and families on learning needs.
• Direct and deliver safe, effective home dialysis therapy.
• Document and communicate lab results and treatment adjustments.
• Assist with emergency response measures and on-call duties.
