Brookdale is seeking a Clare Bridge Program Assistant in Wisconsin to support dementia-friendly activities and resident engagement under the Program Manager, coordinating daily programming and special events.

You'll partner with colleagues to implement person-centered activities, assist with service plan reviews, and help ensure regulatory compliance while fostering a joyful, safe community for residents and families.

Qualifications

• High School Diploma or equivalent is required.
• Associates degree preferred.
• At least one year of experience with older adults or dementia preferred.

Responsibilities

• Supports implementation of person-centered dementia care programs.
• Assists in reviewing residents' service plans and documentation.
• Ensures dementia programs comply with state and Brookdale standards.
• Provides guidance to associates on programming and quality of life.
• Coordinates events and communicates with families about residents' needs.
