Job summary

Brookdale is seeking a Clare Bridge Program Assistant to support person-centered dementia care and engage residents through a calendar of meaningful programs within the Brookdale community. You will assist the Program Manager or Coordinator in delivering activities that honor residents' routines, preferences, and independence.

Responsibilities include leading activities across seven well-being domains, reviewing care plans, ensuring regulatory compliance, and coordinating events.

Qualifications

• High School Diploma or equivalent required.
• Associates Degree preferred.
• Minimum one year of experience with older adults and/or dementia preferred; or equivalent.
• Must have a valid driver's license and a current 3-year driving abstract with no more than one moving violation.

Responsibilities

• Assist in leading person-centered dementia programs across seven well-being domains.
• Review resident service plans and complete required documentation.
• Ensure programs comply with state and Brookdale standards.
• Coordinate and participate in community events including family socials and open houses.
• Support volunteers and provide guidance to associates on programming quality of life.
