Job summary

Brookdale is seeking a Clare Bridge Program Assistant to support the development and delivery of dementia-friendly, person-centered activities for residents. You will work with the Program Manager to implement calendars of activities that promote independence and quality of life within the community.

The role involves coordinating events, assisting with service plan reviews, and ensuring programs comply with Brookdale standards and regulations, while engaging families and staff in meaningful

Qualifications

• High School Diploma or equivalent required.
• Associates Degree preferred.
• Minimum one year of previous experience with older adults and persons with dementia preferred; or equivalent combination of education and experience.

Responsibilities

• Supports the implementation of a person centered program to meet individual needs and interests of the residents.
• Assists in leading and engaging residents in the 7 domains of well-being programs.
• Assists the Program Manager or Coordinator in reviewing individual resident’s service plans and completing documentation.
• Assists in ensuring dementia care programs meet state and Brookdale standards and regulations.
• Participates in special events and communicates with families and residents about the program benefits.
• Helps with volunteer program training and supervision; performs related duties to support care quality.

Education
