Job summary

Brookdale is seeking a Clare Bridge Program Assistant to help lead person-centered dementia-friendly activities and programs within a Brookdale community. You will work under the Program Manager or Coordinator to engage residents in meaningful, individualized activities that support their quality of life and independence.

The role emphasizes collaboration with families and staff, adherence to Brookdale standards, and participation in special events and volunteer programs as needed.

Qualifications

• High School Diploma or equivalent required.
• Associates Degree preferred.
• Minimum one year of experience with older adults and persons with dementia preferred.
• Valid driver’s license and a current 3-year driving abstract with no more than one moving violation required.

Responsibilities

• Assists Clare Bridge Program Manager or Coordinator in leading person-centered dementia-friendly programs.
• Engages residents in the 7 domains of well-being and documents program activities.
• Reviews individual resident service plans and maintains required program documentation.
• Ensures dementia programs comply with state, federal, and Brookdale policies.

Education
