Job summary

Brookdale is seeking a Clare Bridge Program Assistant to implement person-centered dementia programs and engage residents in meaningful activities. This role supports the Program Manager, coordinates events, and collaborates with families to enhance quality of life within the Brookdale community.

Ideal candidates have a High School Diploma or equivalent, experience with older adults, strong communication skills, and a valid driver's license.

Qualifications

• High School Diploma or equivalent; Associate degree preferred.
• Must have a valid driver's license and a clean 3-year driving abstract with no more than one moving violation.
• Ability to lift up to 50 pounds; occasional travel may be required.

Responsibilities

• Supports the implementation of a person centered program for residents' well-being.
• Leads and engages residents in a range of well-being programs across seven domains.
• Reviews residents' service plans and completes program documentation as required.
• Ensures dementia care programs meet state and Brookdale policies and standards.
• Coordinates special events and communicates with families and residents' needs.
