Job summary

Brookdale is seeking a Clare Bridge Program Coordinator to plan, coordinate and facilitate dementia-friendly activities. You will develop a seven-day program, engage residents in meaningful activities, and partner with residents to create monthly calendars, ensuring compliance with Brookdale standards and regulatory guidelines.

The role supports family education and community events while coordinating with leadership and clinical partners to address changes in residents’ health and well-being.

Qualifications

• Bachelor’s Degree in therapeutic recreation, gerontology, education or related field preferred.
• Minimum two years of direct programming experience with older adults and persons with dementia preferred; or equivalent combination of education and experience.
• Must have a valid driver’s license and a current 3-year driving abstract with no more than one moving violation as a condition of employment.

Responsibilities

• Plans, coordinates and facilitates a calendar of person centered dementia friendly programs.
• Provides person centered programs while engaging residents in meaningful dementia care activities.
• Designs a seven day a week program meeting residents' needs and interests.
• Develops monthly calendars in partnership with residents and schedules events.
• Ensures programs comply with Brookdale standards and regulatory requirements.
