Job summary

Brookdale is seeking a Clare Bridge Program Assistant in Jacksonville, FL to support dementia-centered programming and resident well-being. You will assist in leading activities aligned with Brookdale’s Resident and Family Engagement philosophy, engaging residents in meaningful plans while honoring individual habits and preferences.

The role emphasizes safety, program documentation, and collaboration with the Program Manager to deliver high-quality care and engagement within a dementia care

Qualifications

• High School Diploma or equivalent required; Associate degree preferred.
• Minimum one year of experience with older adults and/or dementia preferred; or equivalent education/experience.
• Must have a valid driver's license and a clean 3-year driving record as a condition of employment.

Responsibilities

• Assist in leading person-centered dementia-friendly programs for residents.
• Engage residents in well-being programs across 7 domains (purposeful, intellectual, social, spiritual, emotional, physical).
• Review and document resident service plans per Brookdale standards.
• Ensure programs comply with state/federal regulations and Brookdale policies.
• Coordinate community events, family activities, and open houses.
• Assist with volunteer program and provide training/supervision as needed.
• Perform related duties including personal care, housekeeping, and dietary tasks as required.
