Qualifications

• Proficient with computers, email, and EMR systems.

• Team-oriented with a passion for helping others.
• Experience obtaining and documenting patient vital signs is preferred.
• Healthcare experience in senior living or medical offices is beneficial.

Responsibilities

• Coordinate patient care and follow-up services.
• Manage incoming calls, emails, and patient inquiries.
• Process provider orders, including medications, labs, imaging, referrals, and home health services.
• Communicate care plan updates with patients, families, and providers.
• Support medication reconciliation and escalate concerns as needed.
• Assist with post-discharge outreach and scheduling follow-up care.
• Identify urgent patient concerns and coordinate timely interventions.
• Maintain accurate documentation and patient records.
• Support telehealth appointments and care coordination activities.
• Obtain and document patient vital signs and report abnormal findings.
