Qualifications

• High school diploma or GED is required.
• Medical Assistant is preferred but not required.
• Experience with electronic medical records is desirable.
• Strong communication and customer service skills are essential.

Responsibilities

• Coordinate patient care and follow-up services.
• Handle incoming calls, emails, and patient inquiries.
• Process provider orders including meds, labs, imaging, referrals, home health.
• Communicate care plan updates to patients, families, community staff, and healthcare partners.
• Support medication reconciliation and escalate concerns.
• Assist with post-discharge outreach and follow-up care.
• Identify urgent patient concerns and coordinate timely interventions.
• Maintain accurate patient records and documentation.
• Support telehealth appointments and care coordination activities.
• Obtain and document patient vital signs and report abnormalities.
