Job Description:

We are seeking a dedicated and compassionate Patient Care Coordinator to join our dynamic healthcare team. The successful candidate will play a critical role in ensuring that patients receive the highest level of care and service. You will be responsible for coordinating various aspects of patient care, working closely with medical staff, patients, and their families to facilitate seamless healthcare experiences.

Key Responsibilities:

- Serve as a liaison between patients, healthcare providers, and support staff to ensure effective communication and continuity of care.
- Coordinate patient appointments, follow-ups, and referrals, ensuring timely and accurate scheduling.
- Assist patients and their families in navigating healthcare services, providing information and support throughout the treatment process.
- Maintain up-to-date patient records, ensuring accuracy and confidentiality in compliance with healthcare regulations.
- Work collaboratively with healthcare providers to develop personalized care plans that address patients' needs and preferences.
- Monitor and evaluate patient progress, making adjustments to care plans as necessary in consultation with the healthcare team.
- Facilitate communication among multidisciplinary teams to ensure comprehensive care delivery.
- Provide education and resources to patients and families about diagnosis, treatment options, and post-care support.
- Address and resolve any patient concerns or complaints promptly, escalating issues to the appropriate personnel when required.

Local travel may be required. Reliable transportation is required

Qualifications:

- High school diploma or equivalent; additional education in healthcare administration, nursing, or a related field is a plus.
- Previous experience in a healthcare setting, preferably in patient care coordination or a similar role.
- Exceptional interpersonal and communication skills, with a strong emphasis on empathy and patient advocacy.
- Proficiency in healthcare information systems and Microsoft Office Suite.
- Strong organizational skills with the ability to manage multiple priorities efficiently.
- Knowledge of medical terminology and healthcare regulations, including HIPAA.
- Demonstrated problem-solving ability and a proactive approach to enhancing patient care experiences.
