Western Geriatrics & Neurology is seeking a dedicated Mobile Patient Care Coordinator in Oceanside, CA. You will act as the primary contact for patients, families, and care teams, ensuring timely and coordinated care across settings.

Responsibilities include handling calls and emails, processing provider orders, and supporting telehealth and post-discharge follow-up. The role emphasizes meticulous documentation and compassionate patient interactions to improve outcomes.

Qualifications

• High school diploma or GED required.
• Medical Assistant preferred but not required.
• Strong communication and customer service skills.
• Comfortable using computers, email, and EMR.
• Ability to manage multiple priorities with attention to detail.
• Proactive, team-oriented mindset and passion for helping others.
• Ability to obtain and document patient vital signs.
• Healthcare experience preferred.

Responsibilities

• Coordinate patient care and follow-up services.
• Manage incoming calls, emails, and patient inquiries.
• Process provider orders, including medications, labs, imaging, referrals, and home health services.
• Communicate care plan updates with patients, families, and care teams.
• Support medication reconciliation and escalate concerns to providers when needed.
• Assist with post-discharge outreach and ensure appropriate follow-up.
• Identify urgent patient concerns and coordinate timely interventions.
• Maintain accurate documentation and patient records.
• Support telehealth appointments and care coordination activities.
• Document patient vital signs and communicate abnormal findings to the care team.
