Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, direct, and coordinate hospice care in patients' homes and communities in Akron, OH. You will perform comprehensive assessments, develop and implement care plans, and collaborate with an interdisciplinary team to meet patient and family goals.

The ideal candidate will have an NLN-accredited nursing program credential, current Ohio RN licensure, and experience in hospice, home health, or public health settings.

Qualifications

• Graduate of NLN-accredited nursing program.
• Current state RN licensure.
• Minimum 1 year professional nursing experience in the last 3 years, or a BSN with RN license; 2 years in public health/home care/hospice preferred.

Responsibilities

• Completes initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises the plan of care with patient and family involvement.
• Administers medications and treatments as prescribed.
• Counsels patient and family on nursing and related needs.
• Maintains health care instructions and helps patient with daily living activities.
• Acts as Case Manager for assigned caseload as directed by supervisor.
• Communicates with physicians and other team members to coordinate care.
• Participates in on-call duties and ensures needed equipment and services are available.
• Supervises ancillary personnel when required and pursues ongoing professional development.
