Job summary

Affinity Health Management in Canton, OH is seeking a Registered Nurse to plan, organize, and direct hospice care in home and community settings. The RN will build from community resources to meet the needs of patients and families.

Responsibilities include conducting comprehensive assessments, developing care plans, administering medications, educating patients and families, coordinating with the interdisciplinary team, and participating in on-call coverage.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• RN with 1 year of experience in the last 3 years or a BSN with RN license; 2 years in public health/home health/hospice preferred.

Responsibilities

• Completes initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and initiates plan of care; revises as patient status changes.
• Administers medications and treatments as prescribed.
• Participates in on-call duties and coordinates equipment and services.
