Job summary

Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead patient-centered care in home settings. You will assess, implement, and document plans of care while coordinating with an interdisciplinary team to support patients and families through the hospice journey.

The role emphasizes education for caregivers, symptom management, and timely communication with physicians and the care team, with on-call support as applicable.

Qualifications

• RN with current license in the state of employment.
• Minimum of 1 year RN experience in acute care, hospice or home health; internships considered.
• Hospice or palliative care experience preferred.

Responsibilities

• Assess patients and families and educate them on care plans.
• Deliver skilled nursing care per Plan of Care in home or equivalent setting.
• Coordinate care with IDG and physicians; adjust plans as needed.
• Document care and maintain clinical records; ensure infection control and compliance.
