Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to deliver personalized hospice and palliative care in patients' homes or preferred settings. You will assess, plan, implement, and document care while guiding families through the hospice journey.

Qualifications

• Must be a Registered Nurse with active state license
• Hospice or palliative care experience preferred
• CPR certification required
• Current automobile insurance and valid driver’s license

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings
• Deliver skilled nursing interventions and symptom management per Plan of Care
• Develop and update individualized care plans including visit frequency
• Coordinate care with physicians, social workers, and other IDG team members
• Educate patients and caregivers about disease, treatment, and end-of-life options
• Maintain accurate clinical documentation and ensure infection control compliance
• Participate in on-call and patient support activities as applicable
