Job summary

Gentiva Hospice seeks a dedicated RN Case Manager to lead in-home hospice care, guiding patients and families through every step of their journey. You will deliver skilled nursing, perform assessments, and collaborate with an interdisciplinary team to refine care plans with education and bereavement support.

The role emphasizes compassionate, patient-centered care in home or other settings, with strong emphasis on documentation, regulatory compliance, and coordinating transitions of care.

Qualifications

Current RN license in the applicable state.
Minimum 1 year RN experience in hospice, home health, or related field preferred.
Ability to work in patients' homes and collaborate with an interdisciplinary team.

Responsibilities

Perform comprehensive patient assessments and reassessments.
Deliver skilled nursing interventions and symptom management.
Develop and update individualized care plans with visit frequency.
Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
Provide education and support to patients and caregivers.
