Job summary

Gentiva Hospice seeks an experienced RN Case Manager to deliver compassionate, in-home hospice and palliative care. You will assess patients, coordinate care with a multidisciplinary team, and ensure plans of care reflect evolving needs and goals.

You will educate and support patients and families, document care thoroughly, and uphold regulatory standards while promoting comfort and dignity at every stage of care.

Qualifications

• Current RN license in the state of employment.
• CPR Certification is required.
• Current automobile insurance and a valid driver’s license.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans with appropriate visit frequency.
• Coordinate care across the Interdisciplinary Group (IDG) including physicians and social workers.
• Provide education to patients, families, and caregivers on disease, self-care, and end-of-life care.
• Document timely clinical records and communicate changes in condition to the care team.
