Job summary

Gentiva Hospice is seeking a compassionate Registered Nurse Case Manager to lead in-home hospice care across patients and families. You will perform comprehensive RN assessments, create and update plans of care, and coordinate with physicians, social workers, chaplains, and other team members to ensure relief, comfort, and dignity at end of life.

Qualifications include a valid RN license, CPR certification, and at least 1 year of RN experience in hospice, home health, or acute care; automobile

Qualifications

• Requires RN license and current CPR; minimum 1 year RN experience in hospice/home health is preferred.
• Must have active RN license in state of employment and valid driver’s license.

Responsibilities

• Serve as primary point of contact for patients and families; guide hospice journey.
• Deliver and document skilled nursing care per Plan of Care in patients’ homes.
• Conduct initial and ongoing assessments; adjust care as needed.
• Collaborate with IDG to tailor personalized care plans.
• Provide education, support, and bereavement guidance to patients and families.
