Job summary

Gentiva Hospice is seeking an experienced Hospice RN / RN Case Manager to lead patient care in home settings. You will assess, plan, implement, and evaluate patient care while coordinating with an interdisciplinary team to ensure compassionate, compliant end-of-life services.

The role emphasizes education for patients and families, documentation accuracy, and on-call support as needed. Requirements include RN licensure in the employment state, CPR, and at least one year of RN experience in

Qualifications

• RN license in the state of employment.
• CPR certification.
• At least 1 year of RN experience in hospital, hospice, home health, or related fields.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments in/home settings.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care across IDG including physicians and other professionals.
• Educate patients and caregivers on disease, self-care, and end-of-life needs.
• Document timely clinical records and maintain regulatory compliance.
• On-call participation and responsive patient support as applicable.
