Job summary

Gentiva Hospice is seeking an experienced RN Case Manager to lead patients and families through the hospice journey in home settings. You will deliver skilled nursing care, perform assessments, and collaborate with an interdisciplinary team to update care plans.

This role emphasizes compassionate education, bereavement support, and thorough documentation to ensure high-quality end-of-life care. Applicants should have at least 1 year of RN experience in relevant settings; hospice or palliative

Qualifications

• Minimum of 1 year RN experience in acute care, oncology, long‑term care, hospice or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Lead patients and families through the hospice journey as primary RN contact.
• Provide and document skilled nursing care per Plan of Care at home or other settings.
• Perform initial and ongoing patient assessments; adjust care as needed.
• Collaborate with IDG to refine personalized care plans.
• Offer emotional, physical, and spiritual support to patients and loved ones.
• Educate caregivers on safe care and practical training.
• Communicate updates to the care team for timely interventions.
• Maintain accurate clinical documentation and infection-control compliance.
• Support quality improvement and scheduling initiatives.
• Explain hospice philosophy, services, and end‑of‑life processes to patients and families.
