Job summary

Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to deliver compassionate, patient-centered care in home settings. You will lead assessments, coordinate with an interdisciplinary team, and guide families through end-of-life care with education and support.

This full-time role requires licensure as an RN, CPR certification, and reliable transportation. Experience in hospice or home health is preferred, with a strong emphasis on regulatory compliance and quality care delivery.

Qualifications

• RN license in the state of employment
• CPR certification
• Auto insurance and valid driver’s license
• Minimum 1 year RN experience in hospice, home health, or related settings

Responsibilities

• Perform comprehensive patient assessments and reassessments in person or via telecommunication.
• Deliver nursing interventions and symptom management according to goals of care.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care across IDG with physicians and other providers.
• Educate patients and caregivers on disease, self-care, end-of-life care, medications, nutrition and needs.
• Document timely clinical records and communicate condition changes to the care team.
• Support care continuity across home health, palliative and hospice transitions.
