Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead in-home nursing care for patients with life-limiting illness. You will assess, plan, and implement personalized care, coordinating with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate support for patients and families.

The role emphasizes education for caregivers and effective communication, with responsibilities across documentation, quality, and timely interventions in a home health setting.

Qualifications

• Registered Nurse licensed in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or related.
• CPR certification and valid driver's license required.

Responsibilities

• Perform comprehensive RN assessments and educate patients and families.
• Deliver and document skilled nursing care per individualized Plan of Care.
• Coordinate care across IDG and referrals as needed.
• Maintain timely clinical records and infection control compliance.
• Provide end-of-life care and caregiver education.
