Job summary

Gentiva Hospice is seeking an experienced Registered Nurse to serve as RN Case Manager, delivering in-home hospice and palliative care. You will assess patients, develop plans of care, coordinate with an interdisciplinary team, and educate families while maintaining accurate documentation.

The role emphasizes compassionate, patient-centered care, regulatory compliance, and active participation in on-call and care coordination across home settings.

Qualifications

• Must be a Registered Nurse with current license to practice in the employment state.
• Minimum 1 year RN experience in hospice, home health, or related setting.
• Hospice or palliative care experience preferred.
• CPR certification
• Valid driver’s license and auto insurance

Responsibilities

• Perform comprehensive patient assessments and reassessments in-person and via telehealth.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans with visit frequency.
• Coordinate care across interdisciplinary teams including physicians and other staff.
• Educate patients, families, and caregivers about disease, end-of-life care and medications.
• Communicate changes in condition to appropriate providers.
• Support continuity of care and appropriate referrals between home health, hospice and palliative services.
• Document timely clinical records and ensure regulatory compliance.
• Collaborate with IDG to achieve desired patient outcomes.
• Participate in on-call and patient support activities as applicable.
