Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead patient care in home or community settings, delivering skilled nursing and coordinating with an interdisciplinary team. You will assess, plan, and adjust care to meet each patient’s goals of comfort and quality of life.

You will educate patients and families on hospice services, symptom management, and end-of-life choices, while maintaining accurate documentation and ensuring regulatory compliance.

Qualifications

• Active RN license in the state of employment.
• Hospice or palliative care experience preferred.
• CPR certification required; valid driver’s license and auto insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments in person or by telehealth.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans including visit frequency.
• Coordinate care across IDG with physicians and nurses.
• Educate patients, families, and caregivers about disease, end-of-life care, medications, and self-care.
