Job summary

Gentiva Health Services, Inc. is seeking a Hospice RN / RN Case Manager to lead patient-centered end-of-life care in patients’ homes or alternative settings. You will assess, plan, implement, and coordinate nursing care with an interdisciplinary team to support patients and families with comfort and compassion.

The role emphasizes education for caregivers, accurate documentation, and adherence to regulatory guidelines, ensuring timely interventions and quality assurance across care transitions.

Qualifications

• RN license in the state of employment.
• Minimum of one year RN experience (acute, hospice, home health, or related).
• Hospice or palliative care experience preferred.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care per Plan of Care in home or alternative settings.
• Conduct initial and ongoing assessments and adjust care as needed.
• Coordinate care with interdisciplinary team to refine care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Educate caregivers to provide safe, confident care.
• Communicate patient changes to the care team for timely intervention.
• Maintain accurate clinical documentation and comply with standards.
