Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to travel to patients' homes in Wisconsin, coordinating care with an interdisciplinary team to ensure compassionate, comprehensive end-of-life support. You will assess, plan, implement, and document nursing care while educating families and coordinating with physicians and other professionals.

Candidates should have an RN license, CPR certification, and hospice or palliative care experience.

Qualifications

• RN licensure in state of employment
• Hospice or palliative care experience preferred
• Current CPR certification

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with interdisciplinary team and physicians.
• Educate patients, families and caregivers on disease process, end-of-life care and medications.
• Maintain timely clinical records and ensure compliance with regulations.
