Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to provide in‑home nursing care, comprehensive assessments, and individualized plans of care for patients with life‑limiting illness. You will collaborate with physicians, CNAs, social workers, and chaplains to deliver compassionate, patient‑centered care and education to families.

This role requires a current RN license, a minimum of one year in hospice or related fields, and CPR certification.

Qualifications

• RN license active in the state of employment.
• Minimum of one year RN experience in hospice, home health, oncology, acute care or related settings.
• Hospice or palliative care experience preferred.
• CPR certification.
• Current automobile insurance and valid driver’s license.

Responsibilities

• Be primary contact for patients and families guiding hospice journey.
• Deliver and document skilled nursing care based on Plan of Care in home or other setting.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with IDT to refine care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Teach caregivers with practical training and emotional reassurance.
• Communicate patient condition updates to care team for timely interventions.
• Maintain accurate clinical documentation and infection control standards.
• Support quality improvement and scheduling initiatives.
• Participate in on‑call activities as applicable.
