Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to deliver personalized hospice and palliative care in patients’ homes. You will assess patients, implement and document plans of care, and coordinate with an interdisciplinary team to ensure compassionate, high-quality service.

Responsibilities include education for caregivers, symptom management, and timely communication with the care team while maintaining accurate records.

Qualifications

• Must hold an active RN license in the state of employment.
• Hospice or palliative care experience preferred.
• CPR certification required or in good standing.

Responsibilities

• Perform comprehensive patient assessments and ongoing home visits.
• Deliver and document skilled nursing care per Plan of Care.
• Collaborate with interdisciplinary team to refine care plans.
• Provide education and bereavement support to families.
