Job summary

Gentiva is hiring a Hospice RN / RN Case Manager to lead care for patients in their homes and other settings, coordinating with physicians, social workers, and other care team members. You will assess, plan, and deliver compassionate end-of-life care, supporting families through education and bereavement guidance.

The role requires an active RN license and at least 1 year of hospice or related experience, with a focus on patient-centered care and regulatory compliance across care settings.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or acute care.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person/telephonic visits.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care across interdisciplinary teams and document care plans.
• Educate patients and caregivers on disease, end-of-life care, medications, and self-care.
• Document timely clinical records and maintain regulatory compliance.
