Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to deliver compassionate, in-home nursing care. You will assess patients, implement and adjust plans of care, and coordinate with physicians, CNAs, and social workers to support families through the hospice journey.

You will educate caregivers, document care accurately, and participate in on-call and quality improvement activities to ensure consistent, high-quality hospice services.

Qualifications

• RN license in good standing in the requested employment state.
• Hospice or palliative care experience preferred.
• Knowledge of Medicare/Medicaid/ACHC guidelines and documentation.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update plans of care with visit frequency and risk identification.
• Coordinate with IDG and physicians to refine care plans.
• Educate patients and caregivers on disease, care, and end-of-life topics.
• Communicate condition changes to the care team and document findings.
• Maintain accurate medical records and comply with infection control standards.
• Support on-call activities and provide responsive, compassionate care.
