Job summary

Gentiva Hospice is seeking a dedicated RN Case Manager to lead patient-centered care in patients' homes. You will perform comprehensive assessments, deliver skilled nursing interventions, and coordinate plans with an interdisciplinary team to ensure compassionate end-of-life support.

You will educate patients and families, manage symptoms, document care, and participate in on-call coverage as needed. A current RN license and CPR are required.

Qualifications

• Minimum 1 year RN experience in hospice, home health, or related care.
• Hospice or palliative care experience preferred.
• Current RN license in the state of employment.
• CPR Certification.
• Valid driver’s license and automobile insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments; educate patients and caregivers.
• Deliver skilled nursing interventions and symptom management per care plans.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across IDT with physicians and nurses.
• Educate about disease, self-care, medications, nutrition, and end-of-life care.
• Communicate condition changes to providers and adjust plans accordingly.
• Support continuity of care and transitions between care levels.
• Document timely records including orders, progress notes, and plans.
• Maintain effective IDG relationships and participate in on-call duties.
