Job summary

Gentiva is seeking an experienced Hospice RN / RN Case Manager to lead patient care at the home setting, guided by individualized plans of care and ongoing assessments. You will collaborate with physicians, LPNs, CNAs, and social workers to deliver compassionate end-of-life care, while documenting visits and adjusting plans as conditions evolve.

Qualifications

• Active RN license in the state of employment.
• CPR certification.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic RN assessments.
• Deliver nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care with appropriate visit frequency.
• Coordinate care across interdisciplinary teams and communicate with physicians and providers.
• Educate patients, families, and caregivers about disease process, end-of-life care, medications, nutrition, and self-care techniques.
• Document timely clinical records and maintain compliance with infection control standards.
• Support quality improvement and scheduling initiatives to ensure consistent, compassionate care.
