Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to deliver skilled nursing care in patients' homes or facilities. You will assess needs, develop and update plans of care, coordinate with the IDG, and educate families to support comfort and dignity at end of life.

You'll perform initial assessments, document care, communicate changes to the team, and participate in on‑call coverage as needed. Current RN license and CPR required; hospice or home health experience preferred.

Qualifications

• RN license in good standing
• Hospice or home health experience preferred
• CPR certification required

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care.
• Coordinate care across interdisciplinary teams.
• Educate patients and caregivers about disease, care options, and end-of-life planning.
• Document timely clinical records and maintain compliance.
