Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to deliver patient-centered end-of-life care in patients' homes or affiliated settings. You will assess needs, plan care, and coordinate with an interdisciplinary team to refine personalized plans.

The role emphasizes dignity, comfort, and family education, with emphasis on accurate documentation and adherence to regulatory standards. An RN license in the state is required.

Qualifications

• RN license in good standing in Ohio.
• Minimum 1 year RN experience in hospice, palliative care, home health, or related field.
• Current CPR certification.
• Ability to travel to patient homes as needed.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including home visits.
• Develop and update personalized plans of care with visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Provide education and bereavement support to patients' families.
• Maintain accurate clinical documentation and ensure compliance.
