Job summary

Gentiva Hospice is seeking a dedicated RN Case Manager to lead and coordinate high-quality hospice and palliative care in patients' homes or other approved settings. You will assess needs, implement plans of care, educate families, and collaborate with an interdisciplinary team to ensure compassionate, compliant care.

The role requires an active RN license, at least 1 year in hospice/home health, CPR, and a valid driver’s license, with a focus on patient comfort and family support.

Qualifications

• RN license in the state of employment.
• Minimum 1 year RN experience in hospice or home health.
• CPR certification and a valid driver’s license.

Responsibilities

• Lead patients through the hospice journey and support families.
• Deliver and document skilled nursing care per the Plan of Care.
• Perform initial and ongoing assessments and adjust care as needed.
• Coordinate care with the IDG and physicians.
• Provide education and bereavement support to patients and caregivers.
• Maintain timely clinical documentation and compliance with standards.
• Participate in on-call and care coordination activities.
