Job summary

Qualifications

• RN license in the state of employment and CPR certification.
• Minimum 1 year RN experience in hospice, home health, or related settings.
• Hospice or palliative care experience preferred.

Responsibilities

• Serve as primary contact for patients and families throughout hospice care.
• Deliver and document skilled nursing care per Plan of Care.
• Perform initial and ongoing assessments and adjust care as needed.
• Coordinate care with interdisciplinary team.
• Provide education and bereavement support to families.
• Maintain accurate clinical documentation and infection control compliance.
